    You should receive a courtesy email acknowledging our receipt within 24 hours. Please be sure to have the candidate contact HRS to schedule an appointment for fingerprinting.

    Please be sure to submit only one request. If multiple checks are conducted as a result of submitting the same request more than once, the department will be liable for the costs of the checks conducted. HRS will cover the cost for UF employees, but not for camp volunteers. If the requested check is for a volunteer, please make sure that ‘volunteer’ is indicated at the top of the form.

    If you selected the 435 Livescan option for the purpose of summer camp access, please be sure that the employee/volunteer completes a Clearinghouse Applicant Request form. The completed form should be faxed to Recruitment and Staffing at 352-846-3058 prior to the employee/volunteer’s fingerprinting appointment.
