IRS Form 1095-C demonstrates eligibility for health insurance through your employer. It is not usually necessary to include this form with your annual tax return, but specific circumstances may require it.
1095-C forms for state health plans may be obtained through your People First account at: Insurance Benefits > Health Insurance Tax Forms.
Your People First ID is available in myUFL at: Main Menu > My Self Service > Benefits > PFID & Beneficiary Info.

Historically, the University of Florida has mailed IRS Form 1095-C to all employees who were enrolled in GatorCare or GatorGradCare and eligible for health insurance per the Affordable Care Act (ACA). Beginning with calendar year 2025, these forms will be provided by request only.
