Medical Underwriting Requirements

Medical underwriting requirements are waived for disability insurance and life insurance up to specific coverage amounts during new hire enrollments only. Those guaranteed-issue employee term life insurance amounts are $500,000 for state policies or $300,000 for UFSelect.

For enrollments that exceed those amounts or that are chosen during the annual Open Enrollment, employees must complete an Evidence of Insurability (EOI) form in order to be approved.

For UFSelect plans, that form will be emailed to employees directly by the insurance provider, The Standard, from securemedicalhistory@ssc.standard.com. The linked form will be available for 30 days, after which it will expire. Any incomplete EOI documents will result in employees only receiving the guaranteed coverage amounts.
